Responsibilities

• Conduct comprehensive primary care visits and physical exams in a patient's home
• Create longitudinal care plans for high-risk patients to manage chronic disease
• Partner with an interdisciplinary team to ensure seamless, safe care transitions
• Treat acute needs in the home to prevent ER visits and keep seniors safe and healthy

Compensation

• $104,763 - $166,750 Salary

Requirements

• Master's Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family
• Active, unencumbered NP license in Pennsylvania
• Board Certification (ANCC or AANP)
• Valid driver's license and vehicle for local field-based travel

• Successful completion of a pre-employment drug screening
• Successful completion of a background check
