Responsibilities

• Deliver field-based care, conducting comprehensive primary care visits and physical exams in a patient's home
• Manage chronic disease by creating longitudinal care plans for high-risk patients
• Collaborate with an interdisciplinary team to ensure seamless, safe care transitions
• Treat acute needs in the home to prevent ER visits for seniors

Compensation

• $104,763 + $166,750

Requirements

• Master's Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family
• Active, unencumbered NP license in Pennsylvania
• Board Certification (ANCC or AANP)
• Valid driver's license and vehicle for local field-based travel
• Ability to perform job duties in a manner that ensures the safety of themselves, coworkers, patients, and the public
• Full cognitive and physical functioning at all times
• Compliance with drug and alcohol testing, including for substances that may impair judgment or motor function
• Prohibition of marijuana use (medical or recreational) due to the safety-sensitive nature of the role
• Successful completion of a pre-employment drug screening
