Signify Health, part of CVS Health®, is seeking board-certified Physician Assistant (PA) contractors to conduct in-home health evaluations. This role offers the opportunity to work flexible hours, connect with patients one-on-one, and help them re-engage with their health. PAs will perform medication and medical history reviews and physical evaluations in members' homes, contributing to Signify Health's mission of building trusted relationships to make people healthier.

Practice Info

• In-home visits for Medicare Advantage, Medicaid, and certain commercial plan members
• Pre-scheduled visits in-home with health plan members
• Provided a pre-populated, patient-specific form for each evaluation
• Do not prescribe medicine, order lab tests, or alter people’s current treatment regimen
• Independent Contractor, 1099 status

Responsibilities

• Conduct in-home visits that focus on a person’s overall health and well-being and identify potential barriers to care
• Perform medication and medical history review
• Perform a physical evaluation

Compensation

• Earn up to $1800 to $3000 each week
• Average 6-9 visits daily, base rate of $70 per completed assessment
• Increase your per-visit rate up to 135% with our Market Multiplier
• Earn additional compensation via our pay-per-test model
• Availability Incentives (based on weekly availability)

Requirements

• Board-certified physician assistant/physician associate (PA)
• Medicaid License or willingness to enroll (with our support) strongly preferred
