Ascension Illinois is seeking an experienced Board Certified Nurse Practitioner or Physician Assistant to join their Internal Medicine Department in Chicago, IL. This role offers a unique opportunity to provide 100% outpatient primary care, population health, and immediate care services to Unite Here union members. The successful candidate will partner with Ascension Illinois to deliver compassionate medical services in a busy clinic setting.

• Provide primary care, population health, and immediate care-type services to Unite Here union members (hospitality workers and their adult dependents)
• See 10-20 patients per day on average in the clinic setting

Responsibilities

• Obtain patient history and performs assessment via observation, interview and examination
• Order, perform and interpret diagnostic studies
• Perform preventative health assessments, screening, immunizations and patient care
• Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources
• Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources
• Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy

Compensation

• Salary: $120,419 - $136,370 per year

Shift & Schedule

• 40 hours per week
• Monday-Friday 8:00am - 6:00pm and Saturday 8am - 12pm
• Saturdays and potentially Fridays are rotating

Requirements

• Board Certified Nurse Practitioner or Physician Assistant
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required (American Heart Association or American Red Cross accepted)
• Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required
• Masters Degree of Nursing required
• Bilingual preferred (Spanish, Mandarin or Cantonese, etc)
