Practice Info

• Engage in daily team check-ins, ongoing training, and 1:1 mentorship
• Practice outcomes-based medicine by treating the whole person and addressing social determinants of health
• Lead a sustainable patient panel with full support from an interdisciplinary care team
• Exercise true autonomy to solve problems and do what is right for the patient

Responsibilities

• Deliver field-based care by conducting comprehensive primary care visits and physical exams in a patient's home
• Manage chronic disease by creating longitudinal care plans for high-risk patients
• Collaborate across the continuum by partnering with an interdisciplinary team to ensure seamless, safe care transitions
• Prevent ER visits by treating acute needs in the home to keep seniors safe, healthy, and out of the hospital

Compensation

• Salary: $110,001 - $175,088

Requirements

• Master's Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family
• Active, unencumbered NP license and Board Certification (ANCC or AANP)
• Valid driver's license and vehicle for local field-based travel
• No visa sponsorship available
• Successfully complete a pre-employment drug screening
• Employment is contingent upon successful completion of a background check
