We are seeking a Home Health Occupational Therapist to provide patient-centered therapy services in patients' homes within a welcoming community.
In this role, you will perform comprehensive functional assessments, including home safety evaluations and activities of daily living analysis. You will develop and implement individualized care plans focused on promoting independence, mobility, and safety.
Key responsibilities include training patients and caregivers in the use of adaptive equipment, energy conservation, and fall prevention strategies. You will collaborate with an interdisciplinary team to coordinate care, monitor progress, and adjust treatment plans accordingly.
The position requires a minimum of 1 year of experience, a valid state licensure, and BLS certification. Knowledge of documentation systems like Homecare Homebase and a valid driver’s license are preferred.
