We are seeking an experienced RN - Case Management for a travel assignment in Cary, North Carolina.

- Conducting patient intake, assessments, and symptom screenings following clinic protocols.
- Supporting care coordination among healthcare providers to ensure continuity of care.
- Communicating important clinical updates to providers for timely decision-making.
- Performing medication reconciliation and reporting discrepancies.
- Working 40 hours weekly with 8-hour shifts.
- Providing comprehensive patient care and support.
- 2+ years of nursing experience, preferably in case management or related fields.
- Strong communication and coordination skills.
- Ability to perform assessments and medication reconciliation accurately.
