Role Overview: The Nurse Practitioner provides effective, equitable, value-based primary care to adult Medicare patients. They independently manage office visits, conduct wellness exams, and coordinate care with specialists and support staff. The role emphasizes building patient relationships, health education, care coordination, and participation in population health initiatives.

Key Responsibilities

  • Deliver exceptional primary care and manage routine and acute visits.
  • Perform wellness visits, health assessments, and document comprehensively.
  • Coordinate care with other providers, specialists, and testing facilities.
  • Lead population health efforts, ensuring preventive screenings and outreach.
  • Educate patients on health conditions and treatment plans.
  • Participate in outreach, promotional activities, and home visits as needed.

Qualifications & Skills

  • Master's or Doctorate in Nursing or NP certification in family, adult, or gerontological specialties.
  • Active, non-probationary NP license and DEA registration.
  • US work authorization.
  • Preferred: experience in primary care, understanding of HCC, ICD-10 coding, and bilingual in Spanish.
