Responsibilities Include
Responsibilities Include
- Conducting comprehensive assessments of adult and geriatric patients, reviewing medical history, medications, social factors, and health risks
- Administering and ordering point-of-care and lab tests as needed
- Collaborating with physicians, case managers, social workers, and caregivers
- Providing health education, referrals, follow-up, and documentation according to protocols
- Offering services at patients' homes or care facilities
- Master’s Degree or equivalent experience, with active NP licensure and prescriptive authority
- Board certification in Adult, Geriatric, Family, Emergency, or Acute Care
- Current BLS, ACLS, or CPR certification
- Valid driver’s license and automobile insurance
- Strong communication and computer skills; ability to work independently
- Empathy, trust, and excellent interpersonal skills
