Overview

We are seeking dedicated Physical Therapists to provide individualized, in-home rehabilitative therapy to patients recovering from surgery, illness, or injury. You will conduct comprehensive assessments, develop personalized treatment plans, and adjust care based on evolving patient needs. Practice independently within an interdisciplinary team to help patients regain mobility and independence in their daily environments.

Qualifications & Skills

• Minimum of one year of physical therapy experience or successful completion of a probationary period as a rehab intern.
• Licensure as a Physical Therapist in the state.
• Valid driver’s license required outside Manhattan.
  
  

Key Responsibilities

• Perform comprehensive patient assessments and determine treatment strategies.
• Deliver personalized therapy sessions within patients’ homes or care facilities.
• Continuously evaluate and modify care plans to meet patient needs.
