Role: Licensed Physical Therapist providing one-on-one care to older adults in private homes, senior living communities, and virtual settings.

Responsibilities

• Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy sessions focused on mobility, independence, and wellness.
• Document patient progress accurately in the EMR system and collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
  
  

Qualifications & Skills

• Valid Physical Therapy license or eligibility in practice state(s).
• Degree from an accredited program, basic EMR proficiency, and CPR certification.
• Ability to meet physical demands of community-based care, including lifting, repositioning, and driving.
