- Conducting psychosocial assessments, problem identification, and providing counseling and education to members, families, and caregivers.
- Collaborating with healthcare team members to develop and monitor personalized care plans aimed at enhancing members’ independence and quality of life.
- Facilitating access to community resources, legal processes, and funding programs like Medicaid and Medicare.
- Documenting activities accurately in compliance with organizational standards and analyzing outcomes for continuous improvement.
- Performing related duties such as scheduling family meetings, completing guardianship paperwork, and providing virtual court attendance support.
- Minimum one year of clinical social work experience in healthcare.
- Master’s degree in social work (MSW).
- Licensure as an Independent or Graduate Social Worker in relevant states within six months of hire.
- Experience with Windows-based software and knowledge of community resources.
- Understanding of regional regulatory issues.
