Role: Home Health Speech Language Pathologist (SLP) responsible for providing patient-centered speech therapy services in a home setting.

- Deliver speech, cognitive, and swallowing therapy to patients with communication and swallowing difficulties.
- Conduct assessments, document progress, and ensure patient goals are achieved.
- Educate patients and families on therapy plans, safety, and self-care practices.
- Collaborate with a multidisciplinary home health team to develop and follow patient care plans.
- Support patient recovery and improve communication and swallowing functions.
- Master’s or Doctoral degree in Speech Language Pathology and active Arizona SLP license.
- Minimum of 1 year of speech therapy experience, ideally in home healthcare.
- CPR Certification, valid Arizona Fingerprint Card, and ability to pass background and drug tests.
- Knowledge of home health regulations, critical thinking, communication skills, empathy, and compassion.
