Role: Physical Therapist providing one-on-one care to older adults in private residences, senior living communities, and virtual settings.

Responsibilities

• Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy focused on mobility, independence, and wellness.
• Document care effectively within electronic medical records.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging in place.
• Utilize telehealth/virtual care to enhance access and flexibility.
  
  

Qualifications & Skills

• Valid Physical Therapy license or eligibility in practice state.
• Degree from an accredited program and proficiency with computer and EMR systems.
• Current CPR certification.
• Ability to meet physical demands, including lifting, repositioning patients, and driving.
