Role Overview: The Physical Therapist provides direct rehabilitation care in a skilled nursing environment, supporting patients with long-term and short-term post-acute needs.

Key Responsibilities:

• Develop and implement individualized treatment plans based on physician referrals.
• Assess, modify, and track patient progress towards functional goals.
• Supervise and evaluate care provided by PTAs, rehab aides, and students.
• Maintain accurate documentation and utilize therapy software effectively.
• Collaborate with interdisciplinary teams, communicate with patients and families, and ensure patient safety and rights.
• Follow all laws, regulations, and company policies, including confidentiality and safety standards.
• Participate in community health initiatives when appropriate.
  
  

Qualifications & Skills:

• Graduate from an accredited physical therapy program (MSPT or DPT preferred).
• Active licensure in the applicable state; CPR certification required.
• Excellent communication, organization, and interpersonal skills.
• Proficient in Microsoft Office and therapy documentation tools.
• Ability to make independent decisions, demonstrate good body mechanics, and work effectively within a team.
