Role Overview: The Physical Therapist provides direct rehabilitation care in a skilled nursing environment, supporting patients with long-term and short-term post-acute needs.

- Develop and implement individualized treatment plans based on physician referrals.
- Assess, modify, and track patient progress towards functional goals.
- Supervise and evaluate care provided by PTAs, rehab aides, and students.
- Maintain accurate documentation and utilize therapy software effectively.
- Collaborate with interdisciplinary teams, communicate with patients and families, and ensure patient safety and rights.
- Follow all laws, regulations, and company policies, including confidentiality and safety standards.
- Participate in community health initiatives when appropriate.
- Graduate from an accredited physical therapy program (MSPT or DPT preferred).
- Active licensure in the applicable state; CPR certification required.
- Excellent communication, organization, and interpersonal skills.
- Proficient in Microsoft Office and therapy documentation tools.
- Ability to make independent decisions, demonstrate good body mechanics, and work effectively within a team.
- Preferred one year of experience; new graduates are welcome to apply.
