Role Overview

The Primary Care Physician (PCP) provides comprehensive care for older adult patients with acute and chronic illnesses, ensuring positive health outcomes through accountability, coaching, and care coordination. They lead care teams, manage patient panels up to 450, and collaborate with specialists and hospital staff to deliver high-quality, value-based care.

Key Responsibilities

  • Assess, diagnose, treat, and manage patients, including health promotion and education.
  • Lead and collaborate with care teams, including medical assistants and coordinators.
  • Engage with hospital and transitional care teams for hospitalized or homebound patients.
  • Ensure compliance with clinical guidelines, quality standards, and organizational policies.
  • Contribute to center management, recruitment, and fostering a positive, accountable culture.

Qualifications & Skills

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar; board eligibility required.
  • Current active medical license and DEA registration; BLS certification within 90 days.
  • Strong clinical, leadership, and communication skills; proficiency in evidence-based medicine and lifestyle interventions.
  • Ability to build trust, influence outcomes, and demonstrate self-care for sustainable practice.
  • Willingness to travel locally and nationally as needed; fluency in English.
