- Assessing and evaluating patients' therapeutic, rehabilitative, and functional status within the scope of hospice care.
- Developing and implementing therapy plans aligned with physician orders and supporting patient safety and comfort at home.
- Training patients, families, and caregivers on therapy techniques, safety measures, and device use.
- Documenting care, preparing progress and discharge reports, and participating in care transitions.
- Communicating effectively with interdisciplinary team members and participating in related meetings.
- Supervising Physical Therapy Assistants and instructing home health aides when applicable.
- Bachelor’s or master’s degree in physical therapy from an accredited program.
- State licensure as a physical therapist.
- Current CPR certification.
- Valid driver’s license and insured vehicle.
- Preferred experience in home health or community-based care, with strong organizational and communication skills.
