Role Overview: The Physical Therapist (PT) provides direct rehabilitation care in a skilled nursing facility, focusing on long-term support and short-term rehabilitation for diverse patient populations with orthopedic, neurological, and cardiopulmonary needs.

Responsibilities Include

  • Following individualized treatment plans and establishing functional goals.
  • Overseeing and evaluating care provided by PTAs, rehab aides, and students.
  • Charting accurately and utilizing therapy software effectively.
  • Collaborating with an interdisciplinary team to ensure quality patient care.
  • Maintaining patient safety, rights, and confidentiality.
  • Participating in community health projects and maintaining professionalism.

Qualifications & Skills

  • Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT).
  • Current licensure or registration in the applicable state.
  • CPR certification (obtained upon hire or during orientation).
  • Preferred one year of experience; new graduates are welcome.
  • Proficiency in Microsoft Office and strong communication skills.
  • Ability to make independent decisions, demonstrate professionalism, and work as part of a team.

Key Duties: Implement treatment plans, assess patient progress, supervise staff, ensure documentation accuracy, and provide excellent patient service in a supportive, patient-centered environment.
