Role: A licensed Speech Language Pathologist providing individualized care to older adults in private homes and senior living communities, utilizing the Geriatric House Calls™ model.
Responsibilities: Conduct evaluations, develop treatment plans, deliver evidence-based therapy to improve mobility and independence, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications & Skills: Valid SLP license, degree from an accredited program, basic computer skills, current CPR certification, and ability to meet physical demands such as lifting, repositioning, and driving.
