Role Overview: The Physical Therapist (PT) provides direct rehabilitation care to patients in a skilled nursing facility, focusing on long-term support and short-term rehabilitation for diverse patient needs.

Responsibilities Include

• Implementing and modifying treatment plans based on physician referrals
• Assessing and setting functional goals for patients
• Overseeing care provided by PTAs, aides, and students
• Documenting patient progress accurately and timely
• Collaborating with interdisciplinary teams to ensure quality care
• Maintaining patient safety, confidentiality, and compliance with regulations
• Providing excellent customer service and positive patient interactions
  
  

Qualifications & Skills

• Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT)
• Current state licensure, in good standing
• CPR certification
• Proficiency in Microsoft Office and communication skills
• Ability to make independent decisions, demonstrate professionalism, and work effectively in a team
