Role Overview

The RN Hospital Care Manager I provides comprehensive care management for designated patients across inpatient and outpatient settings.

Key Responsibilities

• Assess patients through screenings and evaluations to identify needs.
• Develop, coordinate, and adjust personalized care plans addressing medical and social factors.
• Support patient self-management via motivational interviewing and coaching.
• Educate healthcare teams and patients, advocating for patient rights and self-care.
• Collaborate with multidisciplinary teams to ensure high-quality, timely care.
• Lead initiatives to improve care transitions and ensure policy compliance.
• Provide education on goals of care for informed decision-making.
• Track metrics, analyze trends, and ensure regulatory adherence.
  
  

Qualifications & Skills

• Current RN license, with a BSN (obtaining within 4 years if not already held).
• Clinical experience in hospital settings and knowledge of disease management.
• Skills in assessment, care planning, motivational interviewing, critical thinking, and communication.
• Proficiency in time management, patient education, and prioritization.
