Role Overview: This position provides care to individuals with substance use disorders in outpatient and inpatient settings, focusing on evaluation, counseling, and therapeutic services.

- Assessing patients’ social, psychological, emotional, financial, cultural, and chemical abuse histories.
- Developing individualized treatment plans and supporting positive outcomes.
- Acting as a liaison with treatment networks and coordinating care with interdisciplinary teams.
- Supporting patients and families through treatment, discharge, and aftercare planning.
- Facilitating court-ordered programming and documenting care per guidelines.
- Providing patient education on addiction, relapse prevention resources, and community support programs.
- Participating in professional development, community education, and in-services.
- Certified Alcohol and Drug Counselor (CADC) and BLS certification within 30 days of hire.
- Bachelor's Degree in a related field.
- Strong communication, assessment, and counseling skills.
- Ability to educate patients and families about addiction and recovery resources.
