Role Overview: The Licensed Practical Nurse (LPN) provides direct patient care in home settings, following physician prescriptions and within the scope of practice defined by state laws.
They document clinical notes, coordinate patient services, and ensure high-quality care by collaborating with the healthcare team.
The LPN operates under supervision from registered nurses or designated supervisors and adapts to diverse home environments while maintaining infection control procedures.

Key Responsibilities
- Deliver clinical services aligned with the care plan and legal scope of practice.
- Accurately document patient visits in electronic medical records and visit verification systems.
- Participate in care coordination, reporting patient needs and condition changes promptly.
- Communicate effectively with patients, families, physicians, and team members.
- Provide care regardless of patient background or circumstances.
- Valid state licensure as an LPN.
- Valid driver’s license, vehicle insurance, and access to reliable transportation.
- Preferred: CPR certification, ability to work flexible hours, strong multitasking, independence, and excellent communication and organizational skills.
