Role: Full-Time Home Health Physical Therapist (PT) responsible for assessing patient needs, developing treatment plans, and performing interventions to improve patient well-being in a home care setting.

Key Responsibilities

• Evaluate patients' functional status, mobility, pain, and medical conditions.
• Assist in developing and implementing individualized therapy plans of care.
• Perform treatment interventions to restore or maintain function and alleviate pain.
• Monitor and report patient responses and progress to the healthcare team.
• Educate patients, families, and caregivers on the care plan and progress.
• Complete patient evaluations, progress notes, and documentation per policies and guidelines.
• Adapt to diverse home environments, ensuring quality care regardless of patient demographics.
  
  

Qualifications & Skills

• Current Physical Therapy licensure in California.
• Valid driver's license, vehicle insurance, or access to public transportation.
• Preferred: CPR certification or ability to obtain within 90 days.
  
  

Compensation & Benefits: Salary ranges from $93,632 to $140,448 annually, based on experience and factors. Benefits include comprehensive health coverage, incentive programs, equity stock purchase, and 401k, subject to eligibility.
