Role: Licensed Physical Therapist providing personalized in-home and senior living care to older adults, utilizing a flexible schedule and incorporating virtual care options.
Responsibilities: Conduct evaluations, develop individualized treatment plans, deliver evidence-based therapies to improve mobility and independence, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and support aging in place through virtual and in-person sessions.
Qualifications & Skills: Valid PT license, degree from an accredited program, proficiency with EMR and computers, current CPR certification, and physical ability to perform caregiving activities including lifting and repositioning patients.
Key Duties: Provide one-on-one therapy in various settings, support patient wellness and functioning, and participate in team-based care. Flexibility to choose employment type—full-time, part-time, or PRN—with benefits and opportunities for professional growth.
