We are seeking an MDS Assessor to join a healthcare facility in Brooklyn, NY.
Responsibilities Include

- Completing accurate assessments and supporting assigned activities
- Creating, distributing, and maintaining monthly care plan calendars and assessments
- Monitoring and auditing clinical records for accuracy and timeliness
- Reporting persistent documentation issues to the Director of Nursing
- Ensuring confidentiality of resident and facility information
- Prior MDS 3.0 experience
- Valid New York RN License
- Experience in Long Term Care setting
- Highly organized, professional, motivated, and friendly
- Strong computer skills and excellent communication abilities
