Role Overview: As a Licensed Practical Nurse, you will provide direct patient care in home settings, following physician’s orders and coordinating with the healthcare team to ensure high-quality services. You will document patient visits, participate in care coordination, and adapt to diverse home environments to meet patient needs.

Key Responsibilities

• Deliver clinical services within legal and practice scope, adhering to infection control protocols
• Accurately document patient visits in electronic health records and visit verification systems
• Participate actively in care coordination, reporting patient needs and communicating effectively with patients, families, and clinicians
• Operate effectively in various home situations, respecting patient diversity and individual needs
  
  

Qualifications & Skills

• Current and unrestricted LPN licensure in the practice state
• Reliable transportation or access to public transit and a valid driver’s license
• Preferred: CPR certification, ability to work flexible hours, manage multiple tasks, work independently, and possess strong communication and organizational skills
