Role: As a Physical Therapist in Home Health, you will assess patient needs related to mobility, endurance, pain, and medical conditions. You will develop treatment plans, perform interventions to improve patient well-being, and monitor progress.

Responsibilities Include

• Evaluating patient functional status and assisting in therapy plan development
• Providing services within legal and professional scope, coordinating with healthcare team
• Monitoring and reporting patient responses and condition changes
• Treating patients to relieve pain, restore function, and maintain maximum capabilities
• Educating patients, families, and healthcare team about care plans and progress
• Visiting patients, documenting visits, and submitting progress notes
• Completing evaluations and developing plans within state guidelines
• Reporting outcomes, goals, and care frequency estimates
• Adapting to diverse home situations regardless of demographic factors
  
  

Qualifications

• Current Physical Therapy licensure in Oregon
• Valid driver's license, vehicle insurance, or access to transportation
  
  

Preferred: CPR certification or ability to obtain within 90 days.
