- Providing services within legal and professional scope, coordinating with healthcare team
- Monitoring and reporting patient responses and condition changes
- Treating patients to relieve pain, restore function, and maintain maximum capabilities
- Educating patients, families, and healthcare team about care plans and progress
- Visiting patients, documenting visits, and submitting progress notes
- Completing evaluations and developing plans within state guidelines
- Reporting outcomes, goals, and care frequency estimates
- Adapting to diverse home situations regardless of demographic factors
- Current Physical Therapy licensure in Oregon
- Valid driver's license, vehicle insurance, or access to transportation
