Key Responsibilities
- Perform tele-health visits, triage, follow-ups, and patient management
- Engage in weekly clinical rounds and team collaboration
- Provide high-quality, evidence-based primary care virtually
- Completed accredited FNP program with certification
- At least 2 years of outpatient primary care or urgent/emergency care experience with patients of all ages
- Strong clinical, communication, and technological skills
- Passion for human-centered care and teamwork
- Adaptability, openness to feedback, and confident in navigating uncertain situations
