Position Overview: A licensed Physical Therapist providing personalized, one-on-one care to older adults in private homes or senior living communities. The role focuses on improving mobility, independence, and quality of life through evidence-based treatments within a flexible schedule that may include telehealth options.

Key Responsibilities

• Conduct evaluations, develop individualized care plans, and deliver therapy sessions to support wellness and functional independence.
• Document patient progress accurately within the EMR system, adhering to regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Utilize telehealth/virtual care to enhance access and continuity of care.
  
  

Qualifications & Skills

• Valid Physical Therapy license or eligibility in the practice state.
• Degree from an accredited PT program.
• Proficiency in computer use and EMR systems.
• Current CPR certification.
  
  

Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, repositioning, and therapeutic activities, and operate a vehicle for home visits. PPE may be required.
