Key Responsibilities
- Conduct evaluations, develop individualized care plans, and deliver therapy sessions to support wellness and functional independence.
- Document patient progress accurately within the EMR system, adhering to regulatory standards.
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff on safety and aging-in-place strategies.
- Utilize telehealth/virtual care to enhance access and continuity of care.
- Valid Physical Therapy license or eligibility in the practice state.
- Degree from an accredited PT program.
- Proficiency in computer use and EMR systems.
- Current CPR certification.
