The Outreach Specialist is responsible for driving program growth through strategic community outreach, relationship development, and prospect engagement. They identify, qualify, and convert referrals into enrollments while educating the community about the PACE care model.

The Outreach Specialist is responsible for driving program growth through strategic community outreach, relationship development, and prospect engagement. They identify, qualify, and convert referrals into enrollments while educating the community about the PACE care model.
Key Responsibilities
- Develop and implement outreach plans to meet growth goals.
- Build and maintain relationships with healthcare providers, community organizations, and referral sources.
- Conduct outreach activities such as presentations and community participation.
- Engage with prospective participants and caregivers to understand their needs and facilitate eligibility assessments.
- Collaborate with internal teams to ensure seamless referral to enrollment processes.
- Maintain accurate documentation in CRM systems and track outreach activities.
- Represent the organization at community events and networking opportunities.
- Use demographic and market data to guide outreach strategies.
- Meet or exceed referral and enrollment targets while applying a person-centered approach.
- High school diploma required; Bachelor’s preferred in Marketing, Communications, Healthcare, or related fields.
- At least 3 years in community outreach, sales, or business development, preferably in healthcare or aging services.
- Experience with CRM systems, Microsoft Office, and public speaking.
- Knowledge of Medicare/Medicaid and senior services is a plus.
- Strong communication, relationship-building, and organizational skills.
- Experience working with the elderly or vulnerable populations is advantageous, with training provided if needed.
- Reliable transportation, valid driver’s license, and auto insurance.
- Up-to-date immunizations and ability to pass background checks.
