After-Hours Hospice Lvn/Rn
We are seeking a compassionate and confident nurse to provide skilled, on-call hospice care during evenings, nights, and weekends.
The role involves delivering professional nursing assessments, symptom management, emotional support, and education to patients and families outside regular hours.
The nurse works independently, maintaining communication with the interdisciplinary team, physicians, and leadership to ensure personalized care plans.
Key duties include responding to urgent patient needs, updating care plans, documenting clinical activities, and upholding hospice standards and policies.
Qualifications include a current Rn or Lvn license, hospice or home health experience preferred, strong clinical judgment, excellent communication skills, and a reliable vehicle with a valid driver's license.
