Role Overview
This position involves leading and coordinating the organization’s efforts to establish and maintain the Patient Centered Medical Home (PCMH) and Patient Centered Specialty Practices (PCSP). The role focuses on implementing continuous process improvements, achieving national recognition, and ensuring quality standards are met without direct patient care.

This position involves leading and coordinating the organization’s efforts to establish and maintain the Patient Centered Medical Home (PCMH) and Patient Centered Specialty Practices (PCSP). The role focuses on implementing continuous process improvements, achieving national recognition, and ensuring quality standards are met without direct patient care.
Key Responsibilities
- Develop and execute action plans for accreditation and recognition compliance.
- Lead process improvement initiatives using nationally recognized methods.
- Coordinate with internal and external stakeholders to support program objectives.
- Monitor data submission, ensure compliance, and streamline processes.
- Serve as a content expert, advising practice sites on recognition opportunities and gaps.
- Educate staff and community partners about PCMH concepts and standards.
- Participate in committees, maintain records, and communicate industry updates.
- Bachelor’s Degree; Nursing preferred, Master’s optional.
- Minimum 2 years related experience; additional experience beneficial.
- RN license in NM or reciprocal; CPR/BLS certification; Specialty certification within 1 year.
- Knowledge of process improvement, clinical standards, and accreditation processes.
- Excellent communication, coordination, and educational skills.
