This role oversees and coordinates case management and discharge planning to ensure high-quality, efficient patient care that complies with regulatory standards.
Responsibilities include: collaborating with multidisciplinary teams to optimize patient flow, ensuring adherence to payer guidelines, and managing discharge processes.
Qualifications: a minimum of three years of case management experience, at least two years in a leadership role, and a nursing degree (RN or BSN). Certification in Case Management is preferred.
Skills required: strong critical thinking, effective verbal and written communication, decisive judgment, proficiency with hospital information systems, and knowledge of regulations and guidelines such as Interqual or Milliman.
Additional expertise: process improvement, performance monitoring, and clinical decision-making based on medical criteria. Ability to work independently in a stressful environment is essential.
